High blood pressure, or hypertension, can have serious health implications if it is not treated properly. It can increase your risk for heart disease, stroke and kidney failure, among other serious health conditions.
With "A Call to Change... Solutions for High Blood Pressure," we can help you create a personal action plan for managing your high blood pressure. Work one-on-one with a registered dietitian or pharmacist to:
- Find out what high blood pressure means for you and your health
- Make food and activity choices that can lower your blood pressure
- Learn about medication and its role in managing blood pressure
- Set realistic goals to improve your overall health
Here's how it works:
- Arrange 15-minute phone sessions with a registered dietitian or pharmacist, depending on the course topic.
- The sessions can be scheduled Monday through Thursday between 7 a.m. and 10 p.m., CST, and Friday between 7 a.m. and 6 p.m., CST.
- You'll receive a course workbook, action planner, food and activity log, and a clinic visit checklist to guide you through doctor visits.
- Complete workbook activities on your own time to learn more about managing your blood pressure and to prepare for phone sessions.
